Dr. Armstrong will call you to answer questions you may have and discuss possible times for an evaluation appointment. Please let her know the best times to contact you to allow you adequate privacy for any personal information you may provide her.

Following the phone call but prior to the initial meeting, Dr. Armstrong will mail you (snail mail!) a packet of forms that you will need to complete and return to her. She has found that the initial meeting is more productive when she has been able to  review your information prior to the first meeting. 

This website includes forms that describe Dr. Armstrong’s practice and policies. Please download and read all forms before you come for your first appointment; Dr. Armstrong will have you sign an acknowledgement of having received the information regarding practice policies at your first meeting.
